| |  | | | | | | Contractor Safety Management System (CSMS) VICO Indonesia has emphasized greatly in developing Health, Safety and Environmental aspects. This is to protect our people at work, the environment and the community. We have also committed to protect our contractors, vendors and customers with the same priority as well as VICO Indonesia employees through CSMS implementation. These guidelines are implemented to help us to better manage the risks by having various Contractors working at sites. All activities in the CSMS are managed in an on-line integrated database system that is accessible to all relevant personnel including Line Managers, Sr. Managers and VP, as well as the President. This system enables the Management to monitor the CSMS activities conducted by all CSMS implementer. There are two phases that consist of six steps each in the CSMS procedure: | | | | | | A. Administration Phase | | | | 1.Risk Assessment The objective is to describe and assess the work and risk. 2.Pre - Qualification It is intended to evaluate whether the contractors are experienced and capable to undertake the activities safely according to VICO HSE standard requirements. The minimum acceptable score for Contractor to pass a CSMS Pre-Qualification is 58.7 for high risk; 43.1 for medium risk; and 33.2 for low risk work. In a case where none of the bidders meet the minimum score, the respective Vice President (VP) will decide to continue or stop the process. 3.Selection The objective of this step is to assess whether the HSE requirements and the bid evaluation criteria have been met. Where necessary, Selection may be performed based on clarification of meetings with successful bidder. The Selection steps are conducted in parallel with bid evaluation process, and considered as part of the technical evaluation. | | | | | | B. Field Implementation Phase | | | | 1. Pre - Job Activity (PJA) consists of two steps: Pre-mobilization and Mobilization activities. Asset Manager or Line Managers shall lead the pre-job activities. 2. Work In Progress (WIP). The objectives of this phase are to ensure that the work performed is conducted according to the HSE requirements and that any additional HSE requirements identified during the work are properly addressed. 3.Final Evaluation (FE).
